Center for Orofacial Pain
There are very few comparable Centers providing comprehensive, coordinated, multidisciplinary care for the orofacial pain patient population. The Center’s activity is medically based rather than dentally based. The multidisciplinary medical management model includes patient education, self-care, medications, physical therapy, behavior modification, biofeedback, acupuncture, and orthopedic appliance therapy. Many of the complex patients are managed in collaboration with the other specialty clinics on the UCSF campus. Acute patients represent about 25% of the patient population, while the remaining 75% are persistent orofacial pain patients including neuropathic pain, such as atypical odontalgia or phantom tooth pain, or patients that are triaged to other facilities.
The Orofacial Pain Center is in the Department of Oral and Maxillofacial Surgery because the management is medical, and it is essential for patients to be managed conservatively before surgical interventions are considered. There is also a close relationship between the Center and pain research projects of Dr. Jon Levine and Dr. Robert Gear. This year the Center space is shared by the Hospital Dentistry GPR program under the direction of Dr. Sophia Saeed.
There are several levels of training occurring in the Center. Lectures are given to first and second year dental students on the subjects of functional anatomy and physiology of the masticatory system, jaw disorders and orofacial pain. Didactic lectures and clinical rotations are offered as elective courses to third and fourth year students, as well as clinic consultations in the pre-doctoral clinics. There is also an opportunity for students in the post-baccalaureate program and International Dental Student program to observe clinical activity and attend post-doctorate lectures on Orofacial Pain.
This Center functions primarily as a fee for service facility, although it has a few special medical contracts with Marin IPA, Hill Physician's Medical Group, and Kaiser Permanente Hospitals.
The faculty continue to collect data on patient treatment outcomes, alternative medicine treatment approaches, and the efficacy of a structured self-management program for chronic myofascial pain patients. The faculty is also an international resource for the development of Orofacial Pain Centers and state of the art multidisciplinary management.
